Guidehouse is seeking an experienced Inpatient Medical Coder to join its team in a full-time, fully remote position.

The Inpatient Medical Coder will review clinical documentation, medical records, and diagnostic results to accurately assign appropriate ICD-10-CM diagnosis and ICD-10-PCS procedure codes. These codes will support billing, regulatory compliance, internal and external reporting, and other healthcare data requirements.

Working under the direction of the Coding Manager, the successful candidate will ensure that medical conditions and procedures are coded accurately and consistently according to ICD-10-CM Official Guidelines for Coding and Reporting, CMS requirements, and other applicable coding standards.

Key Responsibilities

  • Perform accurate inpatient hospital coding across Medicare and non-Medicare accounts, including surgical, medical, cardiac, maternity, newborn, and trauma cases.
  • Maintain up-to-date knowledge of ICD-10-CM, ICD-10-PCS, Coding Clinic guidance, government regulations, official coding guidelines, and third-party billing requirements.
  • Ensure all documented patient services, diagnoses, conditions, and procedures are assigned the appropriate codes.
  • Identify incomplete or insufficient documentation and take appropriate steps to obtain clarification according to facility procedures.
  • Maintain a minimum coding accuracy rate of 95% while meeting established productivity expectations.
  • Maintain an average productivity target of approximately two inpatient charts per hour, subject to client requirements and management guidelines.
  • Follow all facility-specific coding policies, procedures, and documentation standards.
  • Review and manage assigned pending queues daily.
  • Complete GuideAudit review queues and ensure required corrections are properly communicated to the appropriate facility.
  • Submit physician queries when medical records contain conflicting, unclear, or incomplete information regarding reportable conditions or procedures.
  • Follow established physician query policies and Clinical Documentation Integrity (CDI) reconciliation procedures.
  • Correct charts requiring rebilling and promptly communicate necessary changes to the facility.
  • Immediately report system downtime or other issues that could affect required turnaround times.
  • Collaborate with internal quality-control teams to maintain coding standards across assigned facilities.
  • Respond accurately and promptly to coding and billing questions from physicians and hospitals.
  • Code or appropriately pend assigned charts within the required 24-hour turnaround period.
  • Notify administrative staff when circumstances prevent established turnaround requirements from being met.
  • Collaborate effectively with coding and billing teams to support accurate reimbursement for properly documented services.
  • Promptly report coding discrepancies, documentation concerns, or coding-principle issues to supervisors.
  • Maintain professional communication in all workplace correspondence.
  • Accurately record working and billing hours using designated company systems.
  • Work with client and Guidehouse IT teams to resolve technical or system-related issues.
  • Regularly review and use information provided through the Client Portal to support accurate coding and abstraction.
  • Monitor company email regularly throughout working hours.
  • Maintain all required professional certifications and credentials.
  • Complete required continuing education and submit certificates within established deadlines.
  • Maintain HIPAA-compliant workstations and protect patient confidentiality at all times.
  • Follow all applicable Guidehouse and client policies, procedures, privacy requirements, and professional standards.

Required Qualifications

  • High school diploma, GED, or equivalent qualification.
  • At least five years of inpatient ICD-10-CM and ICD-10-PCS coding experience.
  • Must hold at least one of the following professional credentials:
    • RHIA
    • RHIT
    • CCS
    • CIC
    • CPC

Compensation

The annual salary range for this position is $59,000 to $98,000.

Actual compensation may vary depending on factors such as relevant skills, professional experience, training, certifications, licensure, security clearances, and organizational or business requirements.

  • Follow all facility-specific coding policies, procedures, and operational guidelines.
  • Review and manage pending coding queues on a daily basis.
  • Review the GuideAudit queue daily, ensuring assigned charts are processed promptly and any necessary corrections or updates are properly addressed.
